Universal Service Plan for Lifeline Assistance

 

Lumos Networks residential customers may be eligible for Lifeline Assistance, which is a federal government assistance benefit that helps individuals pay for basic, local telephone service.  An eligible customer may receive monthly discounted service on either a wireline or wireless connection, but is available for only one connection per household.  You may qualify for this Plan if you participate in one or more of the following Federal or State Assistance Programs:

 

ó    Medicaid

 

ó    Supplemental Nutrition Assistance Plan (SNAP)

 

ó    Supplemental Security Income (SSI)

 

ó    Federal Public Housing Assistance

 

ó    Low-Income Heat Energy Assistance Program (LIHEAP)

 

ó    National School Lunch Program

 

ó    Temporary Assistance for Needy Families (TANF)

 

ó    Household Income at or below 135% of the Federal Poverty Guidelines
